Endoscopic surgery: Carpal Tunnel Syndrome

Description

Carpal tunnel syndrome is a frequent cause of numbness and weakness of the hand. It is caused by pressure on the median nerve by a tight wrist ligament.  The condition is sometimes, but not always, related to work and strain on the wrists.  Patients often experience, numbness, tingling, pain, and burning sensations of the thumb, index, and middle fingers.  Without proper treatment, the muscles of the hand may weaken permanently.

Treatments

Mild symptoms can be treated by wearing a splint to relieve the pressure on the nerve and with the use of anti-inflammatory medications.  If symptoms persist, or if they are severe, surgery is much more effective at treating the problem.  The surgery involves releasing the ligament and relieving the pressure on the nerve.  Laser treatment is under study, but has not shown serious promise yet.

Surgery

For decades, surgeons have successfully treated carpal tunnel syndrome by making an incision in the palm, finding the tight wrist ligament, and cutting the ligament to relieve the pressure.  They often placed a splint or cast afterwards.  We have learned that the old operation has a difficult recovery, associated with pain, discomfort, and stiffness.  Instead, Dr. Momeni recommends a minimally-invasive technique called endoscopic release.

Advantages of endoscopic release

Endoscopic carpal tunnel release is done with a special instrument that avoids the need for a skin incision on the palm.  The advantage of the endoscopic procedure is a significant reduction in recuperation time and faster return to normal activities. Studies have shown that the pain and weakness of the hand is decreased in the first several months as compared to the "open" traditional method.

The Procedure

The procedure is performed under local anesthesia, with an anesthesiologist in the operating room to administer sedation through an intravenous line (similar to having a colonoscopy).  The surgeon uses a thin isntrument attached to a fiberoptic camera inserted through a small incision at the wrist to release the tight ligament. The procedure takes approximately 20 minutes.

Recovery

Only a light gauze bandage is used, and no cast or splint is used. Patients are encouraged to begin using their fingers and hand immediately following the surgery. The stitches are removed in the office two weeks after the procedure. In most cases, patients notice improvement in their symptoms early after surgery, and the improvement often continues for weeks to months.  Hand therapy may be prescribed during this period.

Results

Overall, surgery is very successful, but the results will depend partly on how long the condition has existed and how much damage has been done to the nerve before the operation.  For this reason, it's a good idea to see a doctor early if you think you may have carpal tunnel syndrome.

Risks

As in any surgical procedure, there is a risk of complications, including anesthesia reaction, bleeding, infection, and nerve or tendon injury. The likelihood of a complication following endoscopic carpal tunnel release is very low. It is very important that your doctor be trained and experienced in the endoscopic carpal tunnel technique and hand surgery in general.
